Me too
I sympathize with you. Mine usually last just two seasons and unfortunately this is the third. I'm going to maybe try coreopsis or rudbeckia next year to see if they aren't hardier in that spot. - Craig
Becky is best
Definitely try 'Becky'. It is an amazing daisy and it keeps on giving year after year. This year, I divided a two year old bunch of it into six good sized plants. I bought another 24 to populate a huge berm, so I imagine I'll be overrun with them in a couple years. They're not invasive. They just like to spread out. When we lived in Chicago, I would divide and give them to all the neighbors every year.
This is also one daisy that looks neat and compact even when it isn't blooming.
Sun
It may be that mine aren't in a sunny enough location. They get maybe 5 hours of sun a day but less in the spring.
Shasta Daisy - Try Becky
The variety that people tend to have the best luck with is Becky. There are a number of other ones out there on the market, some with different flower petal shapes, (see earlier post) and some that change from a pale yellow to white. But my experience and talking to others is they are short lived in our climate.

Look at the roots
If you pull up the dead plant and there is very little root, or the roots are brown and lifeless, root rot over winter and very early spring may be the culprit. Daisies do best in well drained soil, and should be mulched to avoid them being heaved up from the soil in the winter freeze-thaw cycle.
Some varieties of Shasta daisy are hardier than others. The ones mentioned in the daisy article are all very reliable. If you have lost any of those, I would point the finger at poor soil drainage.
